Partners across the NHS, Local Government and wider public and community sector are working together to prevent illness, support people to manage their conditions and to stay independent for as long as possible. In our system, we know that not everyone has an equal opportunity to access healthcare and other services to support their health and wellbeing, and there are disparities between different groups of people in terms of their outcomes.
