This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.

To facilitate safe, supported, early discharge for patients who are returning to the community by providing short term rehab at home for patients who meet the specified criteria.

To work with patients in the orthopaedic wards and identify patients who could be supported at home by the OSDT.

To provide a high standard of care at home for patients requiring short term rehab or awaiting a package of care.

To work on a regular basis without direct supervision from the OSDT Lead. Ability to work alone in patients own home.

Ability to professionally and efficiently record in patients notes following every visit.
